Records an architecture decision as a Nygard/MADR-shaped ADR under decisions/ — capturing the context that forced the choice, the options weighed, the decision itself, and its consequences in both directions, plus the supersedes chain that keeps a decision log honest. Read when asked to record an architecture decision, write an ADR, log the decision we made, document why we chose X over Y, capture this decision for the record, or supersede an old decision with a new one. Do NOT read to frame a proposal or explore an idea not yet decided (frame-a-proposal), to write a spec or implementation plan (write-a-spec), to write an incident postmortem (write-a-postmortem), or to judge whether a design is sound (review-a-design). Use when you have a finalized brainstorm-beagle spec at `.beagle/concepts/<slug>/spec.md` and need a bite-sized, TDD-driven implementation plan before any code is written. Triggers on: "write a plan", "plan this spec", "turn the spec into a plan", "now plan the implementation", "write-plan". Reads the spec, designs the file structure, decomposes work into 2-5 minute TDD steps with exact paths and commands, self-reviews against the spec, gets user approval, then writes to `.beagle/concepts/<slug>/plan.md` and offers to generate an execution handoff prompt via the subagent-prompt skill. Does NOT brainstorm specs, write code, or execute the plan — produces the plan document (and an optional handoff prompt) only.

Use when you need a bite-sized, TDD-driven implementation plan but do NOT have a brainstorm-beagle spec to plan against. quick-plan reconstructs intent from the current conversation, fans out domain-expert exploration subagents across the codebase, and synthesizes the same plan format write-plan produces — without requiring `.beagle/concepts/<slug>/spec.md`. Write the postmortem for an incident, in its incident channel, once it is mitigated or resolved. Use on "write up this incident", "postmortem", "incident summary", "what happened here for people who weren't around", "draft the retro", or when the oncall memory says incidents of this severity get one. Reads the incident channel (and the alert thread it came from), the investigation findings and the data behind them, and writes a one-screen, zero-context write-up: what happened, impact with numbers, detection, timeline with durations, why it happened, what fixed it, follow-ups with proposed owners, and one before/during/after chart. Posted and pinned in the incident channel as its final pinned post, with edit requests updating that one pinned post in place — unless the person or the team's conventions say write-ups live elsewhere, in which case it is written there and only a link is posted, nothing pinned. A person edits and publishes it; never assigns blame.

RFDB server becomes unresponsive (60s+ timeouts) during enricher addEdges write storms. Use when: (1) RFDB CPU spikes to 60-90% after addEdges calls; (2) subsequent reads time out at 60s; (3) server stays catatonic for 5-15 min after a write storm; (4) Tokio main thread parked (write-lock contention). Root cause: add_edges() calls maybe_auto_flush() which has a memory-pressure path that fires on NODE count — not edge count — and holds the exclusive write lock during disk I/O. Fix: separate maybe_auto_flush_edges() uses byte-limit only.
